2015-03-12 560492 1610 CAIB CAIB 2015-03-12 es CAIB text/html 2015-03-10 11224 11225 ![CDATA[Requerimiento de datos bancarios a los interesados en expedientes de prestaciones sanitarias instruidos por el Servicio de Salud de les Illes Balears (Ib-Salut) y cuyo importe ha sido retrocedido por las entidades bancarias a la Dirección General del Tesoro]] 4301 es ![CDATA[<p> <span style="font-family:times new roman,times,serif;font-size:12px;">Una vez comprobado que no se ha podido practicar personalmente el requerimiento individual a las personas interesadas por parte del servicio de correos, y en aplicación del artículo 59.5 de la Ley 30/1992, de 26 de noviembre, de Régimen Jurídico de las Administraciones Públicas y del Procedimiento Administrativo Común, se comunica a las personas mencionadas en el anexo que su entidad bancaria ha retrocedido el pago de la prestación aprobada, por lo que deben presentar <span style="color: black;">un escrito de comunicación de datos bancarios de una cuenta actualmente operativa a nombre de la </span>persona titular de la tarjeta sanitaria, conformado por<span style="color: black;"> la entidad bancaria y firmado por el titular de la cuenta. Si lo desean, tenemos un</span> modelo de dicha comunicación a su disposición en la Unidad de Prestaciones Sanitarias del <span style="color: black;">Servei de Salut de les Illes Balears en Palma, </span>calle de la Reina Esclaramunda, 9. </span></p> <p> <span style="font-family:times new roman,times,serif;font-size:12px;">La documentación requerida se puede presentar de acuerdo con lo señalado en el artículo 38.4 de la Ley 30/1992, de 26 de noviembre, de Régimen Jurídico de las Administraciones Públicas y del Procedimiento Administrativo Común, o en las oficinas del <span style="color: black;">Servei de Salut de les Illes Balears en Palma, ya indicadas.
